연구 배경: 대부분의 임플란트 시스템에서 고정체와 지대주를 연결하기 위해 임플란트 나사를 사용하는데 필요할 때 쉽게 보철물을 제거할 수 있는 장점이 있다. 하지만 임플란트 나사는 적절한 토크로 일정한 조임력을 유지해야 하는데 그렇지 않으면 기계적 합병증이 발생할 수 있다. 치아 임플란트 식립 이후에 많이 발생하는 나사 풀림과 관련한 기계적 합병증을 예방하기 위한 가장 효과적인 방안은 나사 조임 과정에서 토크값을 정확하게 전달할 수 있는 도구를 사용하는 것이다. 이 연구의 목적은 임플란트 시술 과정에서 나사의 조임과 풀림에 사용되는 도구로 목표 토크값을 정확하게 적용할 수 있는 기계식 토크 조절기의 디자인을 개발하는 것이다.
연구 방법: 임플란트 과정에서 나사 조임과 토크 조절기에 관련된 많은 선행 연구의 조사와 분석으로 성공적인 디자인 개발에 필요한 합리적인 디자인 개발 방향을 도출하였다. 설정된 디자인 방향을 토대로 몇 차례의 프로토타입 제작과 컴퓨터를 활용한 시각화 작업으로 도출된 대안을 평가하였으며 최종 개발된 디자인은 씨엔씨 밀링으로 고충실도 프로토타입을 제작하였다.
연구 결과: 디자인 개발 방향에 따라 개발된 가변형 토크 조절기는 힘 전달축과 나사 회전축이 동일선상에 위치하고, 회전축의 길이를 조절할 수 있어서 세밀한 목표 토크값 조절이 가능하며, 부여된 토크값을 시술자가 육안으로 확인할 수 있어 정교한 회전력 전달이 가능한 것이 특징이다.
결론: 스프링식과 마찰식 토크 제한 장치의 특성을 융합한 구조가 특징인 연구 결과는 기존 기계식 토크 조절기의 정확성과 사용성을 획기적으로 개선하였다. 연구 결과인 가변형 토크 조절기는 현재 특허 출원이 완료되었고 실질적인 대량 생산 환경에서 발생할 수 있는 잠재적인 문제들을 검토한 후 관련 기업과 실용화 방안을 모색 중이다.
Background: Most implant systems use implant screws to connect the fixture to the abutment, which has the advantage of making it easier to remove the prosthesis if necessary. However, implant screws must be tightened with the proper torque to maintain a constant tightness, otherwise mechanical complications can occur. The most effective way to prevent mechanical complications associated with screw loosening, which often occur after dental implant, is to use a tool that can accurately transmit torque during the screwing process. The purpose of this study is to develop a mechanical torque controller that can accurately apply targeted torque value as a tool used for tightening and loosening screws during implant procedures.
Methods: The investigation and analysis of many previous studies related to screw tightening and torque controllers during the implantation process led to a rational design development direction for successful design development. Based on the established design direction, alternatives were evaluated using prototypes and computer-generated visualization, and a set of high-fidelity prototype using CNC milling was produced as the final design.
Results: The variable torque controller developed according to the design direction is characterized by the fact that the force transmission shaft and the screw rotation shaft are located on the same axis, the length of the distance can be adjusted to adjust the target torque value, and the applied torque value can be visually checked by the operator, enabling precise rotational force transmission.
Conclusion: Featuring a structure that fuses the characteristics of spring and friction-style torque controller, the research results dramatically improve the accuracy and usability of existing mechanical torque limiting devices. The variable torque controller as a result of research is currently patent-pending and is being explored for commercialization with relevant companies after reviewing potential problems in a practical mass production environment.
