연구배경: 2017년 노인실태조사에 의하면 노인의 89.5%가 의사 진단을 받은 질병을 1개 이상 가지고 있으며 약물이 처방되는 경우에는 하루 평균 4.1개의 약을 복용하는 것으로 나타났다. 노인의 약물 복용 비율이 높아짐에 따라 약물 복약의 문제점도 늘어나고 있다. 노인의 약물 사용 실태 조사 선행연구에 의하면 노인의 약물 오용 유형은 약물 용량 과다사용, 약물 용량 저량사용, 임의로 투약 중단, 투약 시간 불이행 등으로 나타나고 있다. 여기에 더하여 다른 사람의 약물복용, 다른 질병의 약물복용 등, 잘못된 약의 복용 위험도 증가하고 있다. 그러나 의약품을 장기적으로 복용하는 환자의 대다수가 시각 인지능력이 떨어지는 노약자나 고령임에도 불구하고 복약지도는 이러한 상황을 충분히 고려하고 있지 못하고 있다. 따라서 본 연구에서는 만성질환으로 다중 약물을 복용하고 있는 노인을 대상으로 복약지도에서 최종 단계라고 할 수 있는 처방약 약포지에 노인의 특성에 적합한 중요 정보를 전달할 수 있는 디자인 개발 방향을 제시하고자 한다.
연구방법: 현행 약포지의 문제점을 파악하기 위하여 1) 노화로 인한 시각기능에 따른 현상을 살펴보았다. 2) 처방약 조제 형태에 따른 약 포장의 종류를 수집 분석 하였다. 3) 약포지의 시각정보 전달방식 및 문제점을 파악하였다. 4) 만성질환 및 다약제를 복용하는 65세 이상을 대상으로 설문조사 및 문제점을 파악하였다. 5) 3)과 4)를 토대로 약포지 시각 정보의 문제점을 재정립하고 이에 따른 개선 방향을 제시하였다. 6) 텍스트 타입과 그래픽 타입으로 디자인 개선안을 컬러 적용 여부와 함께 제시하였다. 7) 개선된 약포지의 사용자 편의성 검증을 진행하였다. 8) 검증에서 나타난 약포지 디자인 수정사항을 적용하여 디자인을 개선 보완하였다.
연구결과: 현행 약포지의 문제점과 설문조사를 통하여 노인을 대상으로 한 현행 약포지의 시각정보 전달 문제점을 도출하였다. 문제점 파악을 통하여 약포지의 정보를 중요 정보와 보조 정보로 분류하였다. 이후 텍스트, 그래픽으로 표시사항을 세분화하여 복약지도에서 최종단계라고 할 수 있는 처방약 약포지의 디자인 개발 방향을 제시하였다.
결론: 본 연구는 이제까지 처방약 봉투에 국한되었던 약의 정보를 약포지에 표기함으로써 환자들이 약포지만을 보관 또는 휴대할 경우에도 혼동과 불편함을 최소화할 수 있을 것으로 기대한다. 약국이 선택적으로 사용하는 ATC(Automatic tablet counting and dispensing) 의약품전자동조제기의 컬러인쇄 기능의 제약으로 인해 개선된 디자인 적용에 대한 한계점이 있을 수 있으나 컬러가 있는 부분을 선 인쇄하여 사용한다면 해결 가능할 것으로 예상한다. 본 연구를 통하여 노인의 시지각 특성을 고려한 의약품 정보 전달 방식에 도움이 되기를 기대한다.
Background: According to a 2017 survey of elderly people, 89.5% of the elderly have one or more diseases diagnosed by the doctors. And It was found that an average elderly person takes 4.1 drugs were taken per day. According to a prior study of drug use in the elderly, the types of drug misuse in the elderly include overuse of drug dose, use of low dose and non-compliance with medication time. This is because the majority of patients taking the drugs in the long term are elderly people with poor visual cognitive ability, but medication guidance is not fully considering this situation. Therefore, it is intended to present a design development direction that can deliver important information suitable for the characteristics of the elderly on charta, which can be said to be the final step in medication guidance for the elderly who are taking multiple drugs for chronic diseases.
Methods: To identify the problems of the current charta, 1)The phenomenon of the deterioration of visual perception ability of the elderly according to aging was examined. 2)The types of medicine packages were collected and analyzed according to the form of prescription medicine preparations. 3)The method and problems of the visual information delivery of the charta were identified. 4)Surveys and problems were identified for those over 65 who were taking chronic disease drugs and multi-drugs. 5)A charta improvement plan considering the visual perception characteristics of the elderly was presented. 6)Design improvement proposals for text type and graphic type were presented along with whether or not to apply color. 7)User convenience verification of the improved charta was performed. 8)The design was improved and secured by applying the charta design corrections shown in the verification.
Result: Through the problem analysis, information of charta was classified into important information and auxiliary information. After that, the contents of the display were subdivided into text and graphics, and the design development direction of charta, which can be said the final stage in the medication counseling, was presented.
Conclusion : This study is expected to minimize confusion and inconvenience even when patients keep or carry only the charta by making the drug information, which has been limited to the prescription drug envelope, on the chata. Although there may be limitations on the improved design application due to the limitations of the color printing function of the ATC printing device selectively used by the pharmacy, it is expected that it will be possible to solve the problem if the colored part is printed in advance. It is expected that this study will be helpful in the delivery of drug information considering the visual perception characteristics of the elderly.
