목적 : 미숙아 수유불내증에서 erythromycin투여 시, 고용량요법과 중간용량요법의 효과와 부작용을 비교하고자 하였다.
방법 : 부산백병원 신생아중환자실에 입원한 출생체중 1,500 g이하의 미숙아 중 수유불내증이 발생한 40명을 무작위로 두 군으로 나누어 한 군은 고용량의 erythromycin 요법(12.5 mg/kg을 6시간 간격으로 하루 4회, 14일 동안 투여)을 시행하였고, 나머지 군은 중간용량요법(첫 2일은 10 mg/kg을 6시간 간격으로 4회 투여하고, 나머지 5일은 6시간 간격으로 4 mg/kg으로 투여하여 총 7일 투여)으로 투여하였다. 두군 사이의 장관영양과 연관된 변수들인 완전장관영양 도달시간, 수유중단 횟수, 재원기간, 출생체중 회복기간 등을 비교하였고, erythromycin의 부작용 및 정맥영양의 부작용으로 보고된 담즙정체, 유문협착증, 부정맥 등을 관찰하여 비교하였다.
결과 : 두 용량군 사이에 인구통계학적, 임상적 자료들은 차이가 없었다. 완전장관영양 도달시간은 고용량군에서 평균 40.4일, 중간용량군에서 38.6일로 두 군간에 유의한 차이가 없었다. 수유중단 횟수는 고용량군에서 3.1회, 중간용량군에서 2.8회로 두 군간 유의한 차이가 없었으며, 재원기간, 총정맥영양 사용기간 등도 두 군간에 차이가 없었다. Erythromycin투여의 부작용으로 보고된 유문협착증이나 QT간격의 연장은 두 군 모두에서 관찰되지 않았으며, 패혈증, 괴사성장염 등은 두 군간에 유의한 차이가 없었다
결론 : 미숙아 수유불내증의 치료에서 경구용 erythromycin의 중간용량 투여요법은 고용량 투여요법과 효과적 측면에서 차이가 없으면서, 투여기간이 짧고, 투여용량이 적어서 효율적인 방법이라고 할 수 있겠다.Purpose : The objective of this study was to compare the effectiveness and safety of intermediate dose oral erythromycin treatment for feeding intolerance in preterm infants with high dose oral erythromycin regimen.
Methods : A prospective, randomized study in a tertiary referral center of a university hospital was conducted on 40 preterm infants less than 1,500 g, diagnosed as feeding intolerance. The infants were randomly allocated to receive high dose oral erythromycin (12.5 mg/kg, every six hours for 14 days) or intermediate dose erythromycin (10 mg/kg, every 6 hours for the first 2 days, followed by 4 mg/kg, every 6 hours for another 5 days). The times taken to establish full enteral feeding and the numbers of withheld feeds after the drug treatment were compared between the two groups. Potential adverse effects of oral erythromycin and complications associated with parenteral nutrition were assessed as secondary outcomes.
Results : Baseline characteristics were similar between the two groups. There was no significant difference in times to full feeding (40.4 days in high dose group and 38.6 days in intermediate dose group) and numbers of withheld feeding (3.1 in high dose group and 2.8 in intermediate dose group). No significant differences in episodes of sepsis, necrotizing enterocolitis, cholestasis and death were observed between two groups. No episode of arrhythmia and hypertrophic pyloric stenosis were observed in both group. Noninferiority was demonstrated of intermediated dose regimen for efficacy and adverse effect of erythromycin, compared with high dose regimen.
Conclusion : Intermediate dose oral erythromycin treatment does improve feeding intolerance in preterm infants as effectively and safely as high dose erythromycin treatment regimen.
