제네릭 가격정책인 “동일성분 동일상한가“ 방침은 궁극적으로 약품비절감을 목적으로 한다. 가격인하의 근거는 우리나라 제네릭 가격수준이 가중평균가를 기준으로 제 외국에 비해 높게 나타난 권순만(2010)의 연구에 기반한다. 그러나 연구결과의 함의는 제네릭 가격의 수준보다는 고가의 제네릭이 많이 처방되는 현실을 지적하고 있다. 제네릭 진입을 통한 약품비 절감을 위해서는 제네릭의 가격결정구조 뿐만 아니라 사용장려정책이 동반되어야 한다. 제네릭이 오리지널의 사용을 대체하지 않는 이상 제네릭을 통한 약품비 절감은 불가능하다.
본 연구에서 6가지 성분의 제네릭 진입에 따른 약품비 변동분석 결과, 대상 성분은 수량과 청구액이 모두 증가하여 재정절감효과를 찾아볼 수 없었다. 특히 metformin의 수량은 9.3배, glimepiride는 22.1배나 증가하였다. 제네릭 진입후 오리지널의 수량변화를 살펴보면, pioglitazone을 제외하고 모두 증가하였으며 metformin 5.35배, glimepiride 16.48배의 증가폭을 나타냈다. 제네릭 진입에 따른 오리지널의 대체를 나타내는 drug mix는 대부분 0.83~0.97수준으로 그 효과는 미미하였다. 전반적으로 제네릭 진입에 따라 오리지널도 함께 증가하여 해당 성분의 시장확장효과는 극대화된다. 결국 제네릭 진입은 오리지널을 대체하지 못하고 오리지널과 제네릭의 사용이 함께 증가하여 재정절감보다 재정증가를 유발하는 것으로 나타났다. 이를 위해서는 제네릭의 가격인하 규제정책보다는 저렴한 제네릭이 오리지널을 대체할 수 있는 정책의 시급한 도입이 필요하다. 참조가격제는 이에 대한 훌륭한 대안이지만, 반드시 의약품선택의 결정구조가 처방의사에게 약사, 환자로 분산될 때 제네릭을 통한 약품비 절감을 실현화할 수 있을 것이다.General expectation on generics is that they will bring about substantial savings of drug expenditures by substituting their expensive branded counterparts. So far in Korea, not much empirical studies done in this area.
Recent Korean policy intervention on generics is focusing on the price cut under the assumption that lowering price would directly be linked to curtailment of drug expenditures. This study aims to investigate the savings on drug spendings with the introduction of generics. NHI claims data of 6 substances - Pioglitazone, Voglibose, Metformin, Glimepiride, Tolterodine, Clopidogrel - are used to examine the expected impact of generics. Drug spendings of 2009 were compared with those of 2006. Each substance in the study is considered to have experienced strong generic adoption since 2006.
The results are the followings. Generic introduction contributed to about 70%~216% increase in drug expenditures. The quantity effect was positive for all substances with increasing rate of 1.13~3.90. This implies that generic introduction created new market for survival rather than jumped into the original market to compete with. Drug mix effect of generic introduction in all substances in the range of 0.83~0.97, impling that original drugs had been slightly substituted for generics during the period under study.
A salient feature of this study is that after the generic introduction, use of generics has increased along with their original counterpart. In other words, the volume of original drugs hasn't been decreased but increased instead. The only exception was the case of Pioglitazone.
In conclusion, Even when the generics were actively introduced, we could not witness the fall in drug spendings. For drug spendings to be reduced, the policy such as reference pricing system could be considered as an option for the future.
