건강보험에서 의약품에 지출되는 비용은 국민소득수준 증가, 인구 고령화 및 만성질환자의 증가에 따른 의료욕구의 증가와 신의료기술의 발전에 따른 이용 증가 등을 이유로 지속적으로 늘어나고 있다. 지난 2006년 정부는 ??5.3 약제비 적정화방안??을 통해 지속적으로 증가하는 약제비를 절감하고자 하는 강력한 의지를 표명함으로써, 선별등재방식(positive list system), 보험자의 약가협상 절차, 처방형태에 대한 평가 강화 및 유통구조개선 등 대대적인 개혁을 꾀한 바 있다. 약제비 적정화방안이 시행되고 3년 이상이 경과한 현재, 정부 정책에 대한 평가연구를 통해 정책의 효과를 논하여야 하나, 최근 약제비 현황을 살펴보면 정부의 이러한 조치가 기대만큼의 효과를 발휘하지 못하는 것으로 보인다.
우리나라의 약제비 규제는 가격중심의 통제방식으로 이루어져왔다. 약제비 규제는 가격과 수량측면에서 이루어져야 효율적으로 관리될 수 있으며 이런 측면에서 지출예산을 규제하는 것은 하나의 대안이 될 수 있다. 본 연구에서는 외국에서 적용하고 있는 약제비 고정 예산제(fixed budget)의 고찰을 통해 효율적인 약제비 관리방식에 대하여 제언하고자 한다.Recently introduced PLS (positive list system) policy in Korea intends to gain cost effective in drug expenditure, by allowing reasonable prices relative to products' effectiveness. Under the increasingly constrained health budget situation, it is a reasonable policy tool to be used in any country. However, it does not allow one, as contrary to one easily hopes, to control total drug expenditure. In terms of health policy tools for drug expenditure management, if controlling drug expenditure were set as an objective of health policy in one's setting, one may need additional tools. One such tool Korea can think of under the current situation of rapidly rising drug expenditure is the fixed budget for pharmaceuticals.
It would be rather a stringent policy tool from the pharmaceutical industry's point of view, but if one extends the perspective to society level, it can be considered as a reasonable option. In particular, as in Korea where pharmaceutical policy has been mostly in the form of price control, management of drug expenditure can now be extended toward quantity control as well. Fixed drug budget(FDB), in that regard, is an option open to Korea now. If FDB is considered as a possible policy option, a carefully designed impact analysis from all different angles is strongly encouraged. This is because the same policy could have different kinds and level of impacts under different health care systems. No matter what, it is about time for Korea to consider FDB as a drug policy option.
