목 적 : 소아 청소년기에 발생한 하시모토 갑상샘염 환자들을 대상으로 임상양상과 갑상샘 기능 등을 조사하고 관해군과 비관해군의 특성을 비교하고자 하였다.
방 법:1999년 1월부터 2009년 1월까지 가톨릭대학교 강남성모병원 소아 청소년과에서 하시모토 갑상샘염으로 진단된 59명을 대상으로 진단 시 및 치료 중 임상특성, 갑상샘 기능, 갑상샘 항체 양성율 및 성장지표 등을 알아보았고 관해군과 비관해군으로 구분하여 각각의 특성을 비교하였다.
결 과:대상 환자는 59명으로 남아 9명, 여아 50명이었다. 평균 연령은 10.6±2.7세였고 평균 관찰기간은 3.8±3.1년이었다. 갑상샘 질환의 가족력은 8명(13.6%)에서 보였다. 진단 시 주 증상은 갑상샘 종대(77.9%)와 피곤함(38.9%)을 비롯하여 체중증가, 변비, 성장지연, 두통, 신경과민, 추위 못견딤 및 월경장애 등이었다. 진단 시 항갑상샘 항체의 양성률은 anti-TPO Ab가 88.1%, anti-Tg Ab가 76.2%, 둘 다 양성인 경우는 71.1%이었다. 대상 환자들의 진단 시 성장 특성은 Ht-SDS가 0.09±1.25, Wt-SDS가 0.31±2.07, BMI-SDS가 0.97±0.13였고 이들은 최종 추적 시의 성장 특성과 비교 시 각각 차이가 없었다. 추적 관찰 후 관해군은 19명 비관해군은 40명이었고 진단 시 두 군간의 평균연령과 갑상샘 기능에는 차이가 없었다. 두 군 사이에 진단 시와 마지막 검사한 갑상샘 항체가의 비교에서도 통계적 차이가 없었다. 관해군의 진단 시 Ht-SDS는 0.55±1.03로 비관해군의 -0.13±1.30보다 높았으나 체중 및 BMI-SDS는 두 군간에 차이가 없었다. 진단 시 16명에서 정상 갑상샘 기능, 22명에서 현성 갑상샘 저하, 14명에서 보상성 갑상샘 저하, 7명에서 갑상샘 기능 항진을 보여 총 43명(72.9%)의 환자가 진단시 갑상샘기능이상을 보였으며 마지막 방문 시에는 전체 59명의 환자중 53명의 환자가 관해 혹은 정상 갑상샘기능을 보였다.
결 론:하시모토 갑상샘염진단시 72.9%의 환자가 갑상샘 기능이상을 보였고, 추적 관찰 중 32.2%에서 관해를 보였다. 본 연구에서 진단시 관해를 예측할 수 있는 특별한 인자를 찾을 수 없었으나 향후 더 많은 수를 대상으로 한 연구가 필요할 것으로 사료된다.Purpose:We aimed to study the clinical manifestations and clinical course of Hashimoto thyroiditis in children and adolescents.
Methods:We retrospectively analyzed the clinical manifestations and thyroid function in 59 children and adolescents with Hashimoto thyroiditis who were admitted at Kangnam St. Mary’s Hospital between January 1999 and January 2009.
Results:We investigated thyroid function and clinical manifestations of thyroiditis in 50 female and 9 male patients. The mean age of the patients at the time of diagnosis was 10.6±2.7 years, and the mean duration of follow-up was 3.8±3.1 years. The most common complaints at the time of diagnosis were goiter (77.9%), fatigue (38.9%), weight gain, constipation, growth retardation, headache, nervousness, cold intolerance, and menstrual disturbances. Antithyroid peroxidase antibodies were detected in 52 patients (88.1%); antithyroglobulin antibodies, 45 patients (76.2%); and both antibodies, 42 patients (71.1%). The height, weight, and body mass index (BMI) standard deviation scores (SDS) of the patients at the time of diagnosis were not markedly different from the corresponding scores at the patients’ last visit. At the time of the last follow-up, 19 patients were in remission and 40 were in non-remission state. At the first visit, there were no significant differences between the age, thyroid function test, positive rate of thyroid autoantibody, weight, and BMI SDS of patients in the remission group (RG) and non- remission group (N-RG). However, at the time of initial diagnosis, the height (Ht)-SDS of the patients in RG were greater than in N-RG (P=0.037). At the end of follow up, euthyroidism was achieved in 53 patients, 5 patients had compensated hypothyroidism, and 1 patient had overt hypothyroidism.
Conclusion:The patients with thyroid dysfunction at diagnosis were 72.9% and 32.2% of patients were in remission. We could not find out significant predictive factor for remission at the time of diagnosis and further studies with a large number of subjects should be performed.
