연구배경: 폐경 후 여성이나 만성 질환을 가진 환자의 경우 혈청 미네랄과 골밀도간의 연관성이 있는 것으로 알려져 있으나 연구 결과들은 일치되지 않는 결과를 보여주고 있다. 본 연구는 폐경 전과 후 여성을 대상으로 혈청 미네랄과 골밀도 간의 독립적인 연관성을 평가하고자 하였다.
방법: 2004년 3월부터 8월까지 서울에 소재한 1개 대학병원 가정의학과 외래 또는 건강검진세터를 방문한 여성들 중에서 폐경 전 여성 68명과 폐경 후 여성 52명의 지원자들을 연구대상으로 하였다. 연구 대상자들 모두에게 혈청 미네랄과 요추 및 고관절 부위의 골밀도를 측정하였다. 다중회귀분석을 이용하여 골밀도에 유의한 영향을 미치는 교란변수를 조사하고 혈청 미네랄과 골밀도와 관련성을 조사하였다.
결론: 혈청 마그네슘 및 혈청 칼슘과 마그네슘의 비율은 폐경 전 여성에서 골밀도를 반영해 주는 인자로 생각되며, 혈청 마그네슘 자체보다 혈청 마그네슘과 칼슘의 비율이 골밀도 대사와 더 밀접한 관련이 있는 것으로 인자로 생각된다.Objectives: Some minerals in serum have been known to be associated with bone mineral density (BMD) in postmenopausal women or people with certain chronic diseases, but the results of studies have not been consistent. The objective of the present study was to evaluate the independent association between serum minerals and BMD in pre- and postmenopausal women.
Methods: Sixty eight premenopausal, and fifty two postmenopausal women were recruited from out- patient clinics and a health promotion center in a university hospital located in Seoul for this cross-sectional study. All participants completed serum mineral analysis and BMD measurements at the spine and hip. For data analyses, multiple linear regression models were used.
Results: Higher BMD at the 3rd, 4th and total lumbar spine (L-spine) was significantly associated with lower serum magnesium in premenopausal women, but not in postmenopausal women. The ratio of serum calcium to magnesium was significantly and positively associated with BMD at all lumbar sites in premenopausal women, but not in postmenopausal women. Serum calcium tended to have a negative association with BMD at the 3rd, 4th and total L-spine in post menopausal women, but it was not significant (P=0.1).
Conclusion: Magnesium and the ratio of calcium to magnesium in the serum may serve as predictors for BMD in premenopausal women, but not in postmenopausal women.
